Person in Crisis (PIC) Team –

The Person In Crisis (PIC) Team will respond to, Mental and Behavioral health including Suicidal, Domestic Violence, Homelessness, Substance Use, Wellness Checks, Youth-related issues, Elderly Needs, Family/Neighbor Intervention, Fire Fatalities.
This is unfortunately NOT an alternative to police. The Mobile Crisis Team WILL call 911 if the individual who needs treatment is posing a risk to themselves or others.
Rochester Community Mobile Crisis Team –
A mobile psychiatric emergency department for individuals and families within Monroe County. There are no age restrictions. Individuals/family can be seen by mental health professionals in their home, place of employment, school, etc. Services provided include Crisis intervention/stabilization, mental health assessments and rapid linkage and/or re-linkage to treatment providers for individuals who pose no risk to themselves or others. Mobile Crisis will typically respond within 24 hours. If in need of more immediate support, see above.

CDR Supplemental Needs Pooled Trust –
Individuals with disabilities who have too much money to qualify for Medicaid can put money into a special bank account called a Supplemental Needs Pooled Trust. The money put into this account is not counted as income or resources for people with disabilities applying for Medicaid, and can be used for supplemental needs. Although the funds are pooled together in the Trust, CDR maintains individual sub-accounts for each member.

Office for People With Developmental Disabilities (OPWDD)
“Supporting people to live, work, and take part in their community”
The New York State OPWDD is responsible for coordinating services for New York residents with developmental disabilities. It provides services directly as well as through a network of approximately 500 nonprofit service-providing-agencies, with about 80 percent of services provided by the private nonprofits and 20 percent provided by state-run services.

Strong Center for Developmental Disabilities
- Children and Youth with Special Health Care Needs Project – Assists Local Health Departments (LHDs) in providing needed and appropriate services for children with disabilities and their families.
- Community Consultation Program (CCP) – Providing systemic, behavioral, and educational supports to districts, schools, individuals, and agencies
- Health Disparities – Dedicated to improving health for people with intellectual and developmental disabilities through education to current and future healthcare providers, technical assistance, research, and advocacy
- Improving Family Centeredness Together (IFaCT) – Led by the New York State Department of Health. The project focuses on improving the Early Intervention Program (EIP) experience for families and children.
- Mindfulness Programs – Teaches paying attention to experiences in a deliberate way, without judgment. Skills are learned by in-person practice, and assigned practice.
- MyPathNY.org – MyPathNY.org is a free website that helps students and young adults with disabilities across New York State relocate from school or home for employment.
- Project SEARCH® – High school to employment transition programs for 18-21 year olds with moderate disabilities
